Application Requirements:

• Completed Application (may be completed on site)
• Valid State Issued Photo ID or Valid Passport
• Two different proofs of New Haven residency (no personal mail or lease agreements will be accepted. No expired or copied documents will be accepted. Official documents only.).

An $11.00 application fee applies (for adults 17 and over) and a $6.00 fee for children (16 and under) will be assessed at time of application.
